Non-Credit Reporting Online Form

The Office of Institutional Research needs to submit information to the Commissioner's Office on community engagement activities and non-credit courses that take place in each academic year. Any DSC sponsored program should be reported regardless where it is held. The reporting period is from July 1 to June 30. The deadline to submit your report is July 10.

Reporting Instructions:

  1. USHE's guidelines (click here for guidelines): this includes event codes, definitions, and reporting elements
  2. Two ways to provide your data (choose one that is most convenient to you)
    1. Fill out the online form and click "Submit" (One form for each course or activity. If you have 10 activities, you need to fill out the form 10 times.) OR
    2. Record all year round activities using the Excel template provided (click here for template) and submit the Excel file via e-mail to Andrea Brown (abrown@dixie.edu). One template for each academic year, please.

II. ABOUT THE NON-CREDIT COURSE/ACTIVITY

  1. * Non-Credit Course/Activity TYPE (choose only one for this event/course)
    • Community Outreach (O): Programs or courses designed to create interest in enrolling at the institution (i.e., science fairs, info sessions, cultural events, instructional conferences open to the public, etc). These can be targeted at both youth and adults and typically there is no charge for admission.

    • Personal Enrichment (P): Classes offered primarily for personal benefit. These include sports (youth programs), crafts, hobbies, preschool, etc.

    • Supplemental Workforce Education (W): Courses having direct application to jobs in the workforce regardless of the student's personal reason for taking the course. Examples include: Microsoft Office, publishing software, budgeting, web design, etc.

    • Events (E): Institution sponsored or co-sponsored conferences. This does not include conferences for which the institution simply rents out facilities. Admission will typically be charged for these events. Examples include performances, recitals, sports events, etc.
