College contributes to the Utah State Retirement Plan or TIAA/CREF Retirement Plan. Faculty, exempt, and non-exempt grade 19 and above employees are covered by the TIAA/CREF plan. Non-exempt grade 18 and below employees are covered by the Utah State Retirement Plan. The TIAA/CREF plans is a defined contribution plan and the College contributes 14.2% (employee pays nothing). The employee selects the investment choices and is 100% vested. The Utah State plan is a defined benefit plan and the College contributes 14.2% plus 1.5% to a 401k account (employee pays nothing). Total contribution equals 14.9%. The employee is 100% vested in four years and a defined monthly benefit is paid to the employee at retirement. |

College pays approximately 97% of the premium for the employee and employee's family (spouse and dependent children to age 26) for medical insurance through Public Employees Health Plan (PEHP). Employee pays approximately 3% of the premium (per paycheck (paid two times per month) of $3.79 for single coverage, $7.82 for couple coverage, and $10.44 for family coverage. Co-pays for office visits are $20.00 or $25.00 and for prescriptions are 25%, 30% or 50% of a discounted rate. |

College pays approximately 95% of the premium for the employee and employee's family for dental insurance through Public Employees Health Plan (PEHP). Employee pays approximately 5% of the premium (per paycheck (paid two times per month) of $1.16 for single coverage, $1.47 for couple coverage, and $2.13 for family coverage. Co-pays are paid for dental treatment. |

College pays 100% of the premium for a group term life insurance policy. Death benefit is two times the employee's salary to a maximum of $50,000. Spouse and dependent death benefit is $3,000. |

College pays 100% of the premium for long term disability insurance provided by Standard Insurance. Benefit payment begins day 1 of the 6th month after disability occurs and continues until age 65 or until employee is no longer disabled. LTD payment plus Social Security benefit equals 2/3 of regular salary. |

College sponsors a flexible spending plan (Section 125 of the Internal Revenue Code). An employee may elect to have a tax deferred deduction from their paycheck to be used for reimbursement of 'out-of-pocket' medical, dental, vision expenses not covered by insurance. The employee's portion of the medical and dental premium may also be deducted pre-tax. |

The employee and employee's spouse and dependents are eligible for full tuition waiver for credit classes taken at Dixie State College. Fees and other expenses are not included in this tuition waiver. |

Staff employees only - College observes twelve holidays during the calendar year. Eleven of these holidays are designated by the College and the twelfth is selected by the employee as a personal preference day. |

Staff employees only - College grants sick leave in the amount of four hours per paycheck (one day per month). A maximum of six months of accrued sick leave may be carried forward from one year to the next year. College grants vacation leave of twenty-two days a year for exempt employees and from twelve days to twenty-two days a year for non-exempt employees depending on years of service. A maximum of thirty days of accrued vacation leave may be carried forward from one year to the next year. |
