Application Due by May 31st
To apply for the Bachelor's of Science Degree Completion Program in Dental Hygiene, all prerequisites must be met and the following must be received by the May 1st deadline for the fall semester.
BS Dental Hygiene 2012 Application
Provide Official Transcripts from an ADA Accredited Dental Hygiene Program. (See the Transfer Articulation for questions about transfer credits.)
Completion of an AAS, AS Degree or equivalent in Dental Hygiene.
Current active Dental Hygiene License.
Minimum cumulative GPA of 3.0 for Dental Hygiene coursework.
$100 Application Fee paid to the Cashiers Office (receipt must be attached to the application)
*After application is received the applicant MUST meet with Joni Hale, Health Science Advisor, to discuss remaining General Education requirements. Applicant must sign a form acknowledging remaining GE requirements.*

Evaluation of transfer credit is the responsibility of the student. An official transcript is required for college admission, and will go through a review process. However, if the student thinks that a course they have taken at another institution is equivalent to one from Dixie State College, they must do the following:

Obtain a course description and on some occasions a course syllabus from the exact semester the course was taken.

Complete a Course Substitution Form.
