Application

To apply for the Dental Hygiene Program, all pre-requisite requirements must be met and the following must be received by the January 10th, application deadline. Completed application form (print application form)

  • Official transcript showing grades for all pre-requisite classes (unless the pre-requisites were taken at Dixie State College)
  • Confirmation of 100 minimum clinical hours (must be on dentist's letterhead, signed by dentist, and state exact hours)
  • 3 complete reference forms
  • $50 application fee (check or money order only made out to "Dixie State College Dental Hygiene")  
  •  You will be required to complete a background check.  Please click on the button below to obtain a background check online. THIS NEEDS TO BE COMPLETED BY THE JANUARY 10TH APPLICATION DEADLINE.  (Please do not wait until the last minute as this can take up to two weeks to complete.)


 

 

 As part of our selection process, you may be invited for further testing which consists of two tests.  The Dental Knowledge Exam ( a general dental knowledge test) and the HOBET test (Health Occupations Basic Entrance Test).   You may purchase a HOBET/The Net study guide from a local or online bookstore.

You must be available to come to Dixie State College to take these tests sometime during the first two weeks in February.  If you are invited to continue on in the selection process you will be notified by letter as to the exact dates for testing.   

 Individuals should give careful consideration to the mental and physical demands of the program prior to making application. The pressure involved in undertaking the responsibilities of direct patient care should be considered upon applying to the program.

The applicant should understand that many procedures performed by a dental hygienist expose him/her to blood borne pathogens requiring strict adherence to infection control protocols. In addition, it is an important educational experience to participate as a patient in pre-clinic and in the pain control lab.
