If you have been selected for verification for the 2011-2012 school year, please print, complete and sign the appropriate verification worksheet and return the form with signed copies of your and your parents or spouses 2010 Federal 1040, 1040A or 1040EZ tax forms.

• Dependent Student Verification Worksheet :  If you are selected for verification and were required to use your parent's income information on your FAFSA, you will need to complete the Dependent Verification Worksheet.

• Independent Student Verification Worksheet :  If you were selected for verification and you were NOT required to use your parent's income information on your FAFSA, you will need to complete the Independent Verification Worksheet.

• Authorization to Credit Account :  This form is required from all students receiving Financial Assistance at Dixie State College.

• Confirmation of Independent Status :  You must submit this form with the appropriate documentation if you are required to prove Independent Status.

• Request for Re-evaluation :  To request any changes to your financial aid award, (i.e. accepted financial aid awards, cancel financial aid and/or loan awards) or to request additional loan amounts from remaining eligibility of loan award.

• Appeal for Special Circumstances :  If you or your family has special circumstances that need to be taken into account for consideration of financial aid eligibility. You must submit the appropriate documentation with this form before the appeal can be considered for approval.

• Appeal for GPA-Earned Hours : If you did not meet the Satisfactory Academic Progress (SAP) requirements to continue to receive financial aid. You may appeal your financial aid suspension by completing this form. You must submit any trird party supporting documentation with the completed appeal form before it can be reviewed. If your appeal is granted, you must abide by ALL the conditions to remain eligible for future financial aid.

• Appeal for Complete Withdrawal / No Credits Completed : If you completely withdrew from the semester, failed all your courses or did not complete any of your courses, you did not meet Dixie State's Satisfactory Academic Progress (SAP) requirements. You may appeal your financial aid suspension by completing this form. You must submit any third party supporting documentation with the completed appeal form before it can be reviewed. If your appeal is granted, you must abide by ALL the conditions to remain eligible for future financial aid.

• Degree Audit Request : This form must be completed by your Academic Advisor. It is required for all students who wish to received financial aid and have reached the maximum credit limit for their academic program.

• Appeal for Dependency Override : This form is for dependent students who DO NOT meet the federal criteria for "Independent Status" as outlined on the 2011-12 Free Application for Federal Student Aid (FAFSA) but who want their unique circumstances reviewed in order to be considered an independent student for financial aid purposes.

• Student Asset Verification Worksheet : This form is for those who completed their 2011-12 FAFSA and DID NOT answer their personal asset questions.

• Parent Asset Verification Worksheet : This form is for those who completed the 2011-12 FAFSA and DID NOT answer their parent's personal asset questions.

• PLUS Loan Information Form :  If a parent is interested in applying for a Direct Parent PLUS loan, the parent is required to complete the Loan Information Form and return it by mail or fax to Dixie State College. Our contact information is on the PLUS form. 

• Consortium Agreement :  To receive financial aid while attending two schools the same semester, complete and return this form for clearance to our office. This form must be submitted within the first 3 weeks of the semester.

• Summer Request :  To receive financial aid for summer 2011-2012, must submit this form. The form well be available no earlier the March 1, 2012 and it should be submitted no later than June 12, 2012.

• Financial Aid Check List : There are many steps and conditions which must be completed and met before financial aid funds can be disbursed to student. This check list is to assist DSC students through the full financial aid process.

• Consent to Release Student Information : The Family Educational Rights and Privacy Act, FERPA is a Federal law that protects the privacy of student education records. Under FERPA, a student must give written permission to have another individual such as parents, guardian or spouse, etc. access their Dixie State College Information.  If you would like to waive your FERPA rights by allowing a third party to discuss and/or have access to your education records, written permission is required. Please submit the completed form directly to the Dixie State College Registration and Records Office.
